High-speed train crash in Spain, again.

Another terrible accident occurred on a high-speed train in Spain. With 69 people dead, this is the deadliest accident of the year, but another 46 people died in another high-speed train accident earlier this year. So the death toll for both is now 115 people, plus at least 472 wounded people.

That was not supposed to happen; a train is supposed to be a safe mode of transportation, but if you look at the numbers, something is definitely wrong here, perhaps in more than one place. I feel ashamed that this is happening, as I reckon this is the result of a collection of bad management decisions. Only a single accident in a year is awful, but two are unacceptable, yet here we are.

What are we going to do to prevent anything like this from happening again?
